Repetitive Transcranial Magnetic Stimulation Treatment for Holmes Tremor : Case Report

Holmes tremor is a rare condition caused by a central nervous system insult. Representative symptoms are rest, action, and postural tremors of low frequency. We report a 63-year-old female patient with tremors of the head after bilateral thalamic and midbrain infarction. She experienced a stuporous mental state and cranial nerve palsy. Brain magnetic resonance imaging revealed an acute infarction from the thalamus through the dorsal midbrain on the bilateral side. At 1 month after stroke onset, she developed a resting tremor of the head and an exacerbated tremor by posture without a tremor of the extremities. The tremor was low-frequency (3-4 Hz) for the bilateral splenius capitis at rest. Because her symptoms were not improved by clonazepam and levodopa, we performed repetitive transcranial magnetic stimulation (rTMS) for 2 weeks. After treatment, the frequency and amplitude of the tremors were lower. We suggest that rTMS might be a beneficial treatment for Holmes tremor. Further study will be necessary to establish the mechanism of rTMS in Holmes tremor.
